So we offer a variety of financial options that allow you to get the oral health care you need most. In addition, we will gladly file insurance claims on your behalf, answer any questions about your benefits and help you get the most out of your insurance plan.

2022 Member Plan



Annual Premiums


•  Adult (+12): $325
•  Child (under 12) $200

Membership Benefits


•  Regular professional dental cleanings and exams (up to 2 per year)
•  Includes necessary x-rays and children's fluoride treatments
•  Emergency exam during normal business hours, co-pay $20
•  Periodontal maintenance cleanings at $90 co-pay
•  15% off all other dental treatment (when paid at time of service)

Limitations


•  This plan is not insurance and cannot be combined with any other insurance plan or discount program.
•  Plan is active for one year from date of purchase of the primary member.
•  Membership discounts and benefits are not valid at any other dental office.
•  As a courtesy, we will remind patients when they are due for visits, but it is ultimately the patient's responsibility to schedule needed treatment.
•  $100 reactivation fee if plan is not renewed within 30 days of expiration.
•  Cascade Dental Member Plan terms and benefits may be subject to periodic revision.

Cascade Dental will happily work with most every private dental insurance plan available, whether provided by your employer or self-financed.We are preferred providers for the following insurance companies:

•  Delta Dental PPO
•  Delta Dental of Oregon Premier (also known as Moda)
•  Pacific Source
•  Cigna
•  Aetna
•  United Healthcare
•  GEHA plans coordinated by any of the above companies

While visiting our dentist in Medford, Oregon, you can make payments with CareCredit, cash, check or credit card.
